7. Affidavit duly notarized on non-judicial stamp paper of appropriate denomination, stating he/she is a traditional sand extractor belonging to the local community and his livelihood is dependent on the extraction of sand by manual means (if applicable)

8. Previously issued permit/NOC for Sand Extraction (if applicable)

9. 15 years Residence Certificate of the applicant issued by respective Mamlatdar
|
10. Certificate of Registration of canoe issued by Captain of Ports with make and dimensions of canoe
|
11. NOC by Canoe owner for usage of the Canoe (if applicable)
|
12. NOC Permission of the land owner in whose land the extracted sand will be unloaded/stacked and transported (if applicable)
