Renewal of Shop & Establishment ( FORM IV [See rule 6 ] )

Procedure to Apply

1. Register yourself on and login.
2. Fill in the application form.
3. Attach relevant documents.
4. Submit the application and note the acknowledgement no. for application tracking.
5. The employers shall furnish details of the current employees by visiting "" . Download the employees data and Aadhaar consent format. Fill in the data as directed and upload the same on the mentioned website. The above information has to be uploaded failing which, your application may be kept on hold.
6. For autorenewal, the certificate/license will be issued immediately on successful payment
